I/we am/ are interested to sponsor a child aged ………years residing in the HOPAD Child and women Promotion Society …………………………….………………………………. I/we am/are agree to obey all the rules and regulations of HOPAD sponsorship program, and commit to pay RS/US$/EURO……………(In words……………………………………………………………………………….) as monthly support over a period of ………months/years.

I accept that 95% of above amount will be spent for child's cost of living, food, health-care, education and other extra activities from the date of agreement. And 5% will be deposited as reserve fund in the HOPAD Child and Women Promotion Society bank account.

I/we am/are hereby promise to continue the sponsorship grant regularly as per the rules and regulations of HOPAD and inform the HOPAD in case of any change on the above mentioned commitment.
